PERSONAL DATA PROCESSING CONSENT

I have read and hereby give my full and informed consent and authorize the “IVF GROUP” LLC (EDRPOU; National State Registry of Ukrainian Enterprises and Organizations No.: 40280813) pursuant to the Law of Ukraine “On Protection of Personal Data” and pursuant to information that I have provided to the “IVF GROUP” LLC with the intention to process my personal data within the controller’s database “Clients/Patients” (a named aggregate of organized personal data in electronic form and/or in a form of a filing system) that include, but not limited to my name, surname, middle name, passport details, date of birth, place of birth, marital status, health conditions, medical history for the purpose of health protection, medical diagnosis, provision of care, medical treatment or services provided that the data is processed by a health professional or another employee of a health care institution responsible for personal data protection and whom the laws on medical confidentiality apply to, criminal records that pertain to concerns accusations in crimes, court sentences, implementation by the authorities of a state relevant agency as defined by the law with regard to execution of investigations, operations or any other activities related to crime control and anti-terrorism activities, gender identity, etc. for further collection, registration, accumulation, storage, adaptation, change, updating, use, distribution without my prior consent, in case if such distribution is required by the controller’s actions stated below, transfer, depersonalization, destruction by the controller without my prior consent at any time or no later than ten years upon the termination of interaction with the aforementioned controller, and all other actions and/or activities necessary or/and required for the abovementioned data controller, namely the “IVF GROUP” LLC to exercise its functions, professional duties, to perform its professional services, to deliver the products, to engage into professional programs and all other actions as they may deem appropriate and/or necessary under the circumstances that provide for uninterrupted and effective course of actions of the controller. The controller shall undertake to provide protection of the personal data from unauthorized processing, including, but limited to its loss, illegal or accidental destruction, as well as from unauthorized access or use. The violation of the Ukrainian legislation on personal data protection shall lead to liability envisaged and established by the Law of Ukraine. Personal data subjects have the rights to know the sources of collection and location of his/her personal data and reasons for the processing of personal data, to obtain information regarding the conditions and provision of access to his/her personal data, to receive information regarding the processing of his/her personal data, to revoke the consent on the processing of their personal data in writing. A cross-border transfer of personal data shall be allowed to those countries, agencies, institutions, medical centers, companies, organs, bureaus, firms, departments, etc. that act pursuant to and whose privacy laws provide an adequate level of personal data protection that are to the fullest extent in line with the Law of Ukraine “On Protection of Personal Data” or/and in full compliance with the EU General Data Protection Regulation (GDPR).

INTELLECTUAL RIGHTS

The terms and conditions contained herein with regard to protection of any personal non-proprietary rights and proprietary rights shall be governed by and construed in accordance with and pursuant to the Law of Ukraine “On Copyright and Related Rights”. If an effective international agreement, which was approved as binding by the Parliament of Ukraine stipulates other rules and/or provisions than those set forth in the legislation of Ukraine “On Copyright and Related Rights”, the rules and/or provisions of the international agreement shall be applied. You accept the following that these terms and conditions shall apply in full force and effect to your use of this website and by using this website, you expressly accept all terms and conditions herein in full. Other than content you own, which you may suggest in writing to the owner of this website, the “IVF GROUP” LLC (EDRPOU; National State Registry of Ukrainian Enterprises and Organizations No.: 40280813), under these terms and conditions the “IVF GROUP” LLC owns all rights to intellectual property and materials contained in this website and all such rights are reserved.
